    4.PAEDIATRIC PRACTICE POINTS DENGUE FEVER By Maxie Fernandopulle, Consultant Paediatrician.

    With the onset of the monsoon rains, an Influenza like illness (High fever, myalgia,arthralgia and headache), without upper respiratory symptoms is very suggestive of Dengue Fever. Erythema of palms,face and neck are supportive findings.

    The dangerous leakage phase often occurs around the 4th. to 5th. day. A drop in the Platelet count(<100,000 per cu. mm.) and a concomitant rise in the Packed cell volume (>20%) are ominous findings. A pulse pressure of less than 20 mm. Hg. is a further sinister finding.

    During the leakage phase repeat the Platelet count twice daily and the P. C. V. every SIX hours.Start intravenous fluids in appropriate quantities early. The P.C.V. is good guide for the quantity of fluids to be administered.

    Over enthusiastic use of I. V. fluids, and the use of Hypotonic fluid swill precipitate problems.A significant drop in the Haemoglobin and the P. C. V. is indicative of internal haemorrhage warranting blood transfusion.
